Q: How do I fill out my new patient paperwork?

A: New Patient paperwork must be filled out online through the patient portal. Once your appointment is booked, you will receive an email from handsonhealthwellnesscenter@gmail.com so that you can create your patient portal login. Once your login is created, your forms will be available in the portal.

Q: I'm a new patient and I need to book online, but I don't see how.

A: When you click on “New Patient Scheduling” under “Book Online” it will ask you for your information and our staff will contact you ASAP to get you scheduled for the appropriate appointment type and time. You can also click on the book online button in order to schedule an appointment with the chiropractor or just call in to the office to book an appointment with the Nurse Practitioner directly.

Q: I received an appointment reminder, but I need to reschedule. How do I do that through the message I received?

A: If you click on the Request Contact, our office will contact you immediately so that we can address your concern and get you rescheduled for the appropriate time.

Q: Does your office treat children?

A: Yes, we see children for chiropractic care, but we do not see children for primary care services.

Q: Do you accept my insurance?

A: We are In Network with most major insurance carriers. We have patients with insurance, and those who do not have insurance. We will get your insurance information and do a complimentary benefits check. So whether you have insurance or not, we have affordable care plans to suit your individual needs.
