Annual Physicals

Annual physicals with your primary care provider, or wellness visits are considered to be a best practice that establish a measurable baseline of your health. These visits are intended to prevent illness. It is recommended for healthy adults to have an annual physical exam or wellness visit every 1 to 2 years. At Hands on Health Wellness Center, your primary care provider is your ultimate partner in health. You will form a relationship, setting personal goals to keep you engaged in your care and keep you well.

You can expect your annual physical/wellness visit to include: a review of your personal and familial medical history, lifestyle behaviors, vital signs check, physical exam, and laboratory tests. Your provider will also discuss with you which health screenings are recommended for you such as pap smears, mammograms, skin checks, and screenings for colorectal cancer. This is also the time to ask your provider about any health concerns you may have. To further prepare for your visit you can:

  • Write Down Questions - You may have questions or concerns you’d like to discuss with your provider. Write it down so you don’t forget! 
  • Gather Your Personal and Familial Medical History
  • Know Your Medications
  • Bring a list of any prescribed medications you are taking, even if they were prescribed by someone else. Your provider will also want to know if you are taking over the counter medications, vitamins and supplements.
  • Bring Any Other Important Health Data - If you keep logs such as a food diary, blood sugar log or blood pressure log, please bring this with you to your appointment for review. 

 Your provider will take note of all that had been discussed during your visit, creating an overall picture of your health. This allows you to take charge of your health and be prepared for any issues that may arise in the future.
